openai-node v3 → v4 migration guide

The v4 release of the `openai` Node library is a rewrite with several breaking changes for consumer code.

## Client construction
The `Configuration` class and the `OpenAIApi` class have been removed. Construct the client directly using the default export instead.

Before (v3):
    import { Configuration, OpenAIApi } from "openai";
    const configuration = new Configuration({ apiKey: process.env.OPENAI_API_KEY });
    const openai = new OpenAIApi(configuration);

After (v4):
    import OpenAI from "openai";
    const openai = new OpenAI({ apiKey: process.env.OPENAI_API_KEY });

## Method names
API methods now live under resource namespaces instead of being flat methods on the client.
- `openai.createChatCompletion(params)` is now `openai.chat.completions.create(params)`.
- `openai.createCompletion(params)` is now `openai.completions.create(params)`.
- `openai.createEmbedding(params)` is now `openai.embeddings.create(params)`.

## Response shape
Methods return the JSON response body directly instead of an Axios response object. Remove the `.data` accessor.

Before (v3):
    const response = await openai.createChatCompletion({ /* ... */ });
    const content = response.data.choices[0].message.content;

After (v4):
    const response = await openai.chat.completions.create({ /* ... */ });
    const content = response.choices[0].message.content;
